Cervical herniated disc

Neck pain radiating into the arm: treatment of cervical herniated discs by an FMH neurosurgeon.

What is it?

As at the lumbar level, part of a cervical intervertebral disc can escape and compress a nerve root — most often C5, C6 or C7 — or even the spinal cord (cervical myelopathy).

Which symptoms?

Neck pain radiating into the arm or shoulder, sometimes behind the shoulder blade (cervicobrachialgia), often "electric", with tingling in the arm. Weakness of the arm, hand or shoulder may be associated — then seek advice quickly.

Which treatments?

When conservative treatment has failed, surgery relieves the nerve compression: anterior discectomy with a cage, cervical laminectomy, or corpectomy depending on the case.

How does it differ from a lumbar hernia?

The mechanism is the same, but a cervical hernia radiates into the arm whereas a lumbar hernia radiates into the leg. The surgical techniques also differ.

What is cervical myelopathy?

It is suffering of the spinal cord compressed at neck level, which can affect motor skills and balance. It warrants prompt neurosurgical assessment.
